Spruce Grove Community Coach Evaluation Form

Dear Parents,

In order to improve as a youth soccer organization we would like some feedback from parents and players to assess the effectiveness and the abilities of our coaching staff over the past season and our programs in general. We believe that it is important for you to be honest and fair in your assessment. SGSA needs to know what areas may need to be improved, as well as recognize the positive qualities of each of our coaches.

Please note: To ensure fairness all information is confidential and will not be directly shared with the coaches. The appropiated age group director will use your information to assess and evaluate each coach.
